Why Most AI Projects Fail (And How to Avoid It)
There’s a stat that gets thrown around a lot: 85% of AI projects fail to deliver business value. Having spent 25+ years building production systems, and the last few years applying that to AI, I can tell you the number feels about right.
But the reasons might surprise you. It’s rarely the AI itself.
The Three Ways AI Projects Die
1. Starting with the Model, Not the Problem
The most common failure pattern: a team gets excited about a new model, builds a demo, shows it to leadership, and then tries to find a business problem it solves.
This is backwards.
Production AI starts with a clear, measurable problem. “Our support team spends 40% of their time answering the same 50 questions” is a problem. “We should use AI” is not.
What to do instead: Define the problem first. Quantify the cost. Then evaluate whether AI is the right solution. Sometimes a good search engine or a better FAQ page is all you need.
2. Treating It Like a Software Project
Traditional software is deterministic. You write code, it does what you told it, every time. AI is probabilistic. It gives you the right answer most of the time, and confidently wrong answers the rest.
Teams that treat AI like regular software skip the hardest part: evaluation. They build the pipeline, see it work on 10 test cases, and ship it. Then it hallucinates on a customer query about refund policies and suddenly it’s a liability.
What to do instead: Build evaluation into the pipeline from day one. Define what “good enough” looks like. Measure accuracy, latency, and cost. Set up guardrails for when the model isn’t confident. Have a human fallback.
3. The Integration Gap
Here’s where my systems integration background gives me a strong opinion: most AI projects fail at the last mile. The model works great in a notebook. But connecting it to your actual data, your actual systems, your actual workflows? That’s where things break.
Your CRM has 15 years of messy data. Your ERP has custom fields nobody documented. Your support tickets are in three different systems. The AI doesn’t care how smart the model is if it can’t access clean, current data.
What to do instead: Spend 60% of your time on data and integration, 20% on the model, and 20% on the interface. Get the plumbing right first.
What Engineering-First Looks Like
The teams that succeed with AI share a few traits:
- They start small. One use case, one workflow, one department. Prove value, then expand.
- They measure everything. Not just “does it work” but “is it better than what we had.”
- They plan for failure. Every AI system needs a graceful degradation path. What happens when the model is wrong? When the API is down? When the data is stale?
- They treat it as infrastructure. AI isn’t a feature you bolt on. It’s a system that needs monitoring, maintenance, and iteration, just like your database or your network.
The Bottom Line
AI projects fail when they’re treated as science experiments instead of engineering projects. The model is the easy part. The hard part is everything around it: data pipelines, system integration, evaluation, monitoring, and the human processes that need to change.
